Program Purpose

The purpose of Bridge 2 Home is to:

Provide a safe, supportive, and structured environment for youth in crisis

Offer short-term respite for families navigating conflict or crisis

Assess physical, emotional, behavioral, and social needs

Support family stabilization, conflict reduction, and reunification planning

Connect youth and families to needed community, behavioral health, and social support services

Reduce unnecessary foster care or residential placement through early intervention and crisis support

Target Population

Boys ages 12–18

Youth experiencing short-term instability or family conflict

Youth at risk of homelessness or separation from family

Youth needing temporary safety, structure, and support

Youth referred by parents, schools, community agencies, behavioral health providers, or law enforcement (non-detained, non-adjudicated)

Program Elements & Services

1. Intake & Safety Assessment

Same-day intake available

Immediate safety screening

Behavioral and emotional needs assessment

Family interview and support planning

2. Short-Term Stabilization Care

24/7 supportive supervision

Basic needs: meals, clothing assistance, hygiene support

Routine structure focusing on safety, respect, and accountability

Daily check-ins and emotional support

Crisis intervention as needed

3. Daily Routine & Skill Building

Morning routines and positive start activities

School support or structured daytime enrichment

Life skills coaching (respect, responsibility, communication)

Health and wellness activities

Recreational and social activities in a supervised environment

4. Family Support & Reunification Planning

Conflict mediation

Solution-focused family meetings

Parenting support and resources

Guidance on navigating community systems and referrals

Collaborative reunification plan before discharge

5. Behavioral Health Coordination

(Not therapy unless provided by licensed professionals)

Behavioral observations and documentation

Care coordination with mental health providers

Support connecting families to long-term mental health services

Crisis prevention strategies and coping skills coaching

6. Referral & Resource Navigation

Community mental health support

School support & advocacy

Housing and stabilization resources

Youth mentoring and aftercare services

Family support programs and local agencies

Program Length

Youth may stay up to 14 -21 days

Extensions may occur only if required by safety review and agency guidelines

Discharge and reunification planning start at Day 1
